Official Transcripts and Replacement Diplomas
The Official Transcript Request Form must be completed and signed.
The form may be emailed to asmith@pia.edu, faxed to (412-466-0513), mailed in (PO Box 10897, Pittsburgh, PA 15236), or hand-delivered (5 Allegheny County Airport, West Mifflin, PA 15122). Current rates for official transcripts and diplomas can be found on the form.
All transcript packages will include the official document(s) requested and a copy of the corresponding course descriptions.
